Please print the forms you need and deliver, mail (PO Box 88, Buffalo MN 55313), or fax (763.684.1967) them to the Zion office.
- Facility Use Application Form (pdf) (for both Zion programs and rentals)
- Fundraising Information Form (pdf) and guidelines (for activities that will raising funds or collecting donations in-kind)
- Publicity Request (pdf) (for Zion related programming that will be publicized by Zion)
